ABOUT US The Primary School is expanding the boundaries of traditional education. Our holistic model weaves together education, health, and family support services to ensure underserved children have the system of care they need to lead meaningful and emotionally, mentally, and physically healthy lives. The Primary School was founded in 2015, and we received seed funding by the Chan Zuckerberg Initiative, a major national philanthropy.Our approach has three defining characteristics:We start early. We start working with parents and enroll children into our program as early as birth to reach them at their most critical stage in development. We work with - not around - families. We know that parents and caregivers are the most important people in a child’s life. We partner with them to enhance the education and health of their children. We integrate multiple systems to serve the whole child. Our unique model unites health care and education, building a multi-disciplinary team and holistic system of care around a child’s full needs. In 2016, we opened our flagship school in East Palo Alto, California, with our first class of four-year-olds. We have grown to serve nearly 300 children and their families via a 0-3 parent/child program, a preschool and elementary school.Our ultimate goal is to create a game-changing system of care for children that can be replicated across the country. To that end, we are building a model that can be effectively implemented in diverse communities and sustained on public funding. In 2020, we plan to both expand our student and family population in East Palo Alto and pursue opening our next site in Hayward, California. It’s an exciting time to join the team!ABOUT THE ROLEOur vision is that each child is deeply known across a range of domains, including health, academic, soul, and family. We hope that knowledge is used to deliver a personalized and optimal educational and health experience that is fully coordinated across over school, health care, and home. A core innovation of The Primary School (TPS) model is a highly integrated, team based approach to care that takes into account the needs of the whole child and family.The Manager of Health and Integrated Supports role will work across programs to develop and deliver our approach to physical, mental, and socio-emotional health goals in Hayward.The Manager will support the program planning and operations of the health and integrated supports teams in our efforts to build a novel health-education integrated model. This includes working with the TPS school team, TPS parent coaching team, and local health care providers to reimagine how well child care, developmental services, and social supports are delivered. The Manager will play a key role in ensuring the Medical Director’s work and health team’s vision run smoothly and efficiently at the school and within the parent support programs. They will also help to integrate other components of our school model (e.g. parent coaching, specialized services, mental health) in a seamless manner for families.The Manager will also continue to manage and participate in design and learning initiatives that support EPA and broader dissemination, with a focus on areas of overlap with the Hayward component of the role.This role co-reports to the Managing Director of Program in Hayward. The Manager will also work closely with our Medical Director who oversees the partnership with Kaiser Permanente (KP).RESPONSIBILITIESProgram Design and Implementation:Support research and design of evidence-based elements of our integration and health models. Help to establish processes to facilitate more integrated clinical care services. Collaborate across functions in Hayward to implement care pathways for children with complex needs including health and developmental delays. Support building and executing on our multi-tiered system of care, learning from and with the Hayward site, including overseeing the COST process, external referral process, Success Plans, and Children’s Circles. Manage operations and administrative tasks related to the program. Partnership engagement:Work closely with the Medical Director and Hayward leadership to nurture our partnership with KP and other community health partners who are part of a child or family’s team. Own and drive relationships with the on-the-ground KP Sleepy Hollow to ensure programming is implemented to fidelity. Coordinate and support relationships with other health and social service providers in the Hayward community. Direct service:Provide some direct services to families and/or children as tied to the above goals, including ASQ screening, care coordination, mental health consultation and referrals. Case managing the planning and support for children who require a deeper level of support and/or coordination across multiple partners. Learning and design:Hone new innovations in Hayward with a particular focus on the team based approach to care, health, mental health, and developmental supports, partnering with similar roles in East Palo Alto. Launch 1-3 small learning pilots at Hayward. Partner with the learning team to drive research and program evaluation for elements of the health and education model. Codify our practices, program elements, and lessons learned both to guide internal efforts and to share externally.
